Care Solutions is a partnership of nurse practitioners who provide primary care to homebound adults in private residences and assisted living facilities. We specialize in providing comprehensive service with a focus on treating the whole patient with early detection of acute and careful management of chronic medical conditions. Since we come to you we are better able to address your medical issues before they become urgent, reducing your stress and the need for hospital admission and costly emergency care.
